- an employee (if you are still accruing pension due to disability)
- a former employee (if you are no longer accruing pension with the former NN CDC; also choose this option if you are still working at NN or GSAM but are now accruing pension with BeFrank or NN)
- a Pensioner (if you are already receiving a pension)
- Your employer:
